Робота з ейфорійною пам’яттю у адиктів
Анотація
Стаття концептуалізує феномен адиктивної ейфорійної пам’яті як стійкі, емоційно насичені спогади про вживання психоактивних речовин із дисоціативним характером переживань та дефіцитом символічного рівня. Феноменологічно можна виділити два модулі: (1) гострі «ейфорійні інтрузії» із вегетативними реакціями та потягом; (2) латентну «пам’ять утраченого раю» з виразним компонентом туги та нарцисичним змістом. На нейробіологічному рівні узагальнено мультисистемну модель пам’яті (гіпокамп-контекст/стимул-стимул, амігдала-стимул-афект, дорсальний стріатум-стимул-реакція) і роль стрес-медіаторів та глутамат-залежної пластичності у консолідації та підтриманні адиктивних слідів пам’яті, що уможливлює реактивацію потягу навіть через роки ремісії. Критично проаналізовано обмеження підходів реконсолідації (залежність ефектів від «сили» й «віку» пам’яті, контексту та тривалості інтервенції) і неоднозначні результати експозиційних методик у лікуванні адикцій. Запропоновано альтернативну терапевтичну стратегію проконсолідації як доведення адиктивного досвіду до рівня семантизованого життєвого наративу та посилення кортіко-кортікальної інтеграції. Стратегія містить три послідовні модулі: символізацію/менталізацію мотивацій уживання (зокрема через малюнкові та образні техніки) та наративізацію (конструювання історії від третьої особи з перерозподілом афектів у часовій перспективі). Підкреслено показання: стабільна ремісія (переважно ≥2–3 роки для хімічних адикцій); для поведінкових адикцій можливе більш раннє застосування. Ілюстративні клінічні кейси демонструють операціоналізацію технік і потенціал зміни від суб’єктивної «ейфорійної інтрузії» до інтегрованого досвіду.
