INFLUENCE OF BEHAVIORAL AND PSYCHO-EMOTIONAL STRESS-POTENTIAL FACTORS DEPENDING ON THE LEVEL OF STRESS IN PATIENTS WITH CEREBROVASCULAR PATHOLOGY

Abstract

To assess the impact of behavioral and psychoemotional stress-potentiating factors depending on the level of vulnerability to stress in patients with cerebrovascular pathology in the dynamics of its development, 383 patients with cerebrovascular pathology of various degrees of manifestation. These degrees were examined to determine the target of medical and psychological assistance (main group): from a high risk of developing the disease due to the presence of cardiovascular risk factors to a vascular catastrophe in history. The study included 122 patients, who were characterized by the presence of cardiovascular diseases with a high risk of developing cerebrovascular disease (group 1); 134 patients with clinical manifestations of cerebrovascular disease in the form of a transient ischemic attack in history (group 2) and 127 patients who had ischemic cerebral stroke (group 3). The inclusion criteria for the study participants were: a high risk or a clinically expanded picture of cerebrovascular disease, which developed against the background of cardiovascular diseases in the form of hypertension and ischemic heart disease, verified by a clinical laboratory method; lack of psychiatric and substance abuse history, disorders of consciousness and psychotic states at the time of inspection. The exclusion criteria were: a history of mental and behavioral disorders, severe concomitant somatic pathology (state of decompensation), severe somatic diseases (except cardiovascular and cerebrovascular disease), which may affect the mental state of patients. As a result, the work revealed a higher level of stress load in patients with cardiovascular risk and cerebrovascular disease compared to conditionally healthy examined patients, which indicates a prognostically greater stress vulnerability of this category of patients and draws attention to the importance of the role of stress in the development of the disease. In the genesis of (psycho) somatic disorders, behavioral and psycho-emotional factors were important, with the latter predominating in the development of pathology. The great importance of stress for cerebrovascular disease indicates the need for medical and psychological assistance in the treatment of such patients.
