Визначення вестибулярної функції у дітей хворих на гнійний середній отит
Анотація
Мета дослідження – оцінка вестибулярної функції дітей з гострим гнійним середнім отитом методом комп’ютерної статичної стабілометрії порівняно з типово-розвиненими однолітками. Матеріали та методи. Обстежено 52 дитини віком від 4 до 7 років, 22 дитини з гнійним середнім отитом та 30 практично здорових однолітків методом статичної стабілометрії на пристрої «МПФІ стабілограф 1» (Харків, Україна). До уваги бралися показники варіації та форми розподілу центру тиску, спектрально-кореляційні показники стабілограм, а також інтегральні показники стабільності (довжина, швидкість та кут постуральних коливань) та якості функції рівноваги за двох сенсорних станів (відкриті та закриті очі) розраховані у програмному забезпеченні StabiliS. Результати. Гнійний середній отит викликає постеріорне зміщення абсолютних координат центру тиску. У стані з закритими очима діти з гнійним отитом відрізняють від своїх здорових однолітків за 14 з 24 стабілометричних параметрів, у тому числі за довжиною стабілограми, швидкістю, кутом та амплітудою коливань, поступаючись їм у стійкості рівноваги. Діти з гнійним середнім отитом більш чутливі до зорового каналу постурального контролю, ніж їх однолітки. Залучення зорової аферентації до постурального контролю істотно покращує підтримання балансу у дітей з гнійним отитом за рахунок зменшення девіацій, постуральних коливань, що знаходить відображення у вірогідних змінах 9 з 24 стабілометричних параметрів порівняно з контролем. Висновок. У дошкільному віці гнійний середній отит призводить до прихованої вестибулярної дисфункції, яка виявляється порушенням стійкості вертикальної пози у стані з закритими очима та компенсується за рахунок зорово-вестибуло-пропріоцептивної інтеграції в процесі постурального контролю.
