Механізми впливу низькоінтенсивного оптичного випромінювання на систему мікроциркуляції (огляд)

Анотація

У роботі представлений огляд ефектів та механізмів дії низькоінтенсивного оптичного випромінювання лазерів і світлодіодів на молекулярному, клітинному, тканинному та організменному рівнях.

Відомо, що всі процеси, що протікають в живих організмах на будь-якому рівні їх організації пов’язані з переносом і перетворенням енергії. Причому, в абсолютній більшості цих процесів первинним джерелом енергії є світло. Дія оптичного випромінювання на рослини і тварин різні і пов’язані з відповідними специфічними світлочутливими молекулами, зміною їх конформації і фізіологічної активності, що призводить до зміни метаболізму на клітинному рівні. Дія світла пов’язана також з метаболічними сигнальними мережами, які викликають реакцію на рівні тканини і організму в цілому. В даний час оптичне випромінювання широко використовується в традиційній медицині як складова комплексу лікувальних і реабілітаційних заходів, в фізіотерапії і в ветеринарії, зокрема, при лікуванні інфекційних захворювань, хвороб шкіри, сухожиль, зв’язок, суглобів, пролежнів, захворювань дихальних шляхів, постраневих процесів , порушень росту і розвитку, в лікуванні гіпербілірубінемії. Останнім часом велика увага приділяється терапевтичному використанню неруйнівного й нетоксичного НІОВ лазерів (low level energy lasers, LLEL) і світлодіодів (light-emitting diodes, LED).

Низькоінтенсивне оптичне випромінювання (НІОВ) прискорює загоєння ран, має протизапальну і болезаспокійливу дію, в тому числі в якості для постоперативного анальгетика, має захисну дію на клітини, перешкоджає їх загибелі від цитотоксичних факторів. Інтенсифікація системи мікроциркуляції під дією оптичного випромінювання може бути ключовим механізмом загоєння ран і зменшення болю. Цитопротекторну дію НІОВ різних довжин хвиль і доз на клітини в культурі було показано в цілому ряді робіт, в тому числі на клітини, оброблені отрутами: ціанідом, тетродотоксином, ротеноном і MPP +. Цитопротекторну дію НІОВ і зниження апоптозу було зафіксовано в нейронах, оброблених бета-амілоїду.

В останні роки зростає інтерес до терапевтичного використання непошкоджуючих неінвазивних методів, які практично не мають негативної післядії та протипоказання, до числа яких належить випромінювання світлодіодів (LED). В цьому випадку механізми дії фотонів пов’язують з клітинними і субклітинними рецепторами, фотоіндуковане збільшення або зниження активності яких призводить до каскаду реакцій на клітинному і тканинному, а потім і на рівні організму

Можна виділити наступні встановлені механізми дії НІОВ на тканинному рівні:

1) фотохімічні - енергія оптичного випромінювання поглинається світлочутливими молекулами, а потім передається на молекулярному рівні іншим молекулам і молекулярним структурам, які беруть участь в сигналізації та регуляції метаболічних процесів. Найбільший ефект для кожної світлочутливої молекули надає опромінення з діапазоном хвиль, відповідних максимуму її поглинання.

2) фототермічні - світлоіндукована гіперемія тканин (light-induced hyperthermia), термічні динамічні реакції (thermic dynamic reactions), а при великій інтенсивності – світлоіндукованного випаровування (photovaporisation), виборчий термолізіс (selective photothermolysis), коагуляція (lightinduced coagulation) і злиття тканин (tissue fusion).

3) фотомеханічні - хімічна і термічна абляція (chemical and thermal photoablation), фрагментація (photofragmentation) і руйнування (photodisruption).

4)  фотофізичні - піроелектричний ефект (pyroelectricity).

Вибір оптимального спектрального діапазону, оптимальної дози і режиму опромінення є одним з найбільш важливих питань низько інтенсивної фототерапії. В даний час об’єктивної інформації про механізми, закономірності та результати впливу НІОВ на тканини і клітини недостатньо для розробки і впровадження конкретних (строго регламентованих) клінічних методик, тому фототерапія проводиться як додаток до стандартних терапевтичним і реабілітаційних заходів.

У статті також детально описано поширення оптичного випромінювання в біологічних тканинах як анізотропних багатошарових матеріалах на прикладі математичних моделей.
